Social Worker – Transitional Rehabilitation Program

Fixed-Term Full-Time/ Part -Time (up to 76 hours per fortnight!) 3-Month Secondment Opportunity

An exciting internal opportunity is available for a Social Worker to join our Acquired Brain Injury (ABI) Transitional Rehabilitation Team on a 3-month fixed-term full-time basis. 

This role provides a unique opportunity to work with people aged 18 to 64 years living with an acquired brain injury, supporting their transition and rehabilitation journey within a multidisciplinary environment.

About the Role

As a Social Worker within the Transitional Rehabilitation Program, you will:

• Complete initial social work assessments to identify client goals and support networks.
• Provide ongoing support to clients, families, and significant others throughout the rehabilitation process.
• Assist with Administration and Guardianship matters, including submissions to the State Administrative Tribunal (SAT).
• Support clients with financial assessments, budgeting, and other legal or advocacy-related matters.
• Work closely with the Welfare Assistant to ensure coordinated and holistic support.
• Contribute to multidisciplinary goal setting, discharge planning, and overall case coordination for transition care clients.
