Care Partner Lead – Brightwater At Home

Brightwater is seeking an experienced, values driven leader to step into the role of Care Partner Lead. This is an exciting opportunity for a proactive professional who is passionate about supporting people to live well at home and committed to delivering high-quality, person-centred care within the Support at Home Program.

About the Role

As Care Partner Lead, you will play a pivotal leadership role in supporting and guiding Brightwater’s Care Partner Team. You will be responsible for ensuring exceptional client experiences, strong compliance, and continuous improvement across our Support at Home Services.

This role combines people leadership, operational oversight, and stakeholder engagement to ensure clients receive consistent, responsive, and meaningful support aligned with Brightwater’s values.

Key Responsibilities:

  • Lead, mentor, and support a team of Care Partners to deliver high-quality, person-centred care.
  • Foster a culture of accountability, collaboration, and continuous improvement.
  • Ensure strong understanding and effective delivery of the Support at Home Program, funding models, and client pathways.
  • Monitor and manage team performance and KPIs, including funding utilisation, Care Management Targets and client engagement activities.
  • Promote compliance with the Strengthened Aged Care Quality Standards, Aged Care Act and Legislation, Brightwater policies, and best practice care management.
  • Identify risks, escalate issues appropriately, and implement sustainable solutions.
  • Contribute to service improvement initiatives that enhance efficiency, quality, and client outcomes.

About You:

You are a confident and compassionate leader who thrives in a fast‑paced environment and brings clarity, structure, and energy to your role.

You will bring:

  • Demonstrated leadership experience within aged care, home care, or community services.
  • Strong knowledge of the Support at Home Program, aged care legislation, Strengthened Aged Care Quality Standards quality standards, funding and framework.
  • Experience leading and developing high‑performing teams within a client‑focused environment.
  • Sound understanding of care planning, compliance, and risk management.
  • Proven ability to drive client satisfaction and continuous improvement.
  • Well‑developed problem‑solving and decision‑making skills.
  • Excellent written and verbal communication skills, with the ability to build trust with clients, families, staff, and external partners.
