The Opportunity

We are currently seeking a highly organised and customer-focused Administration Officer to support our Transition Care Community (TCC) Program.

This is an excellent opportunity for an experienced administrator who enjoys working in a fast-paced healthcare environment and takes pride in delivering exceptional service and accurate administrative support.

Reporting to the TCC Service Lead, you will provide dedicated administrative support to the Transition Care Community program, ensuring the smooth coordination of referrals, client onboarding, reporting, records management and communication processes

What you’ll bring  

  • Experience in an administration, coordination or program support role.
  • Strong organisational skills with the ability to manage multiple priorities and deadlines.
  • High attention to detail and accurate data entry capabilities.
  • Excellent written and verbal communication skills.
  • Strong Microsoft Office skills and confidence learning new systems.
  • A proactive approach to problem-solving and process improvement.
  • The ability to work collaboratively within a multidisciplinary healthcare environment.
  • Previous experience in healthcare, aged care, community services or a similar environment will be highly regarded.
  • Experience with aged care systems or PRODA is desirable but not essential.
  • A satisfactory National Police Clearance issued within the past 12 months.
  • A current valid WA driver’s licence.
