This role will be at our Transitional Rehabilitation Programme (TRP) AT Marangaroo.  

Marangaroo (TRP) 

This service is based in Marangaroo. The TRP program provides residential based community rehabilitation to clients.  The service provides 24/7 support for individuals living with acquired brain injuries and other neurological disability (eg. Huntington’s Disease, Parkinson's Disease, Multiple Sclerosis, FND)  including those with mental health needs and other co-morbities with complex cognitive, behavioural, and physical needs. This can include  people with  tracheostomy, Prolonged Disorders of Consciousness, TBI, stroke, neurodegenerative conditions 

Patients are referred from a Perth Metro Hospital including the State Rehabilitation Services at Fiona Stanley Hospital for continued slow stream rehabilitation.  

The service delivers a highly individualised, person-centred and goal oriented program through a multidisciplinary team, combining clinical support, rehabilitation programs, and behavioural strategies to maximise independence and quality of life. With a secure, accessible environment, the program focuses on maximising the client’s independence and potential including meaningful community engagement; working closely with families and stakeholders to ensure a coordinated, holistic approach to each client’s wellbeing prior to discharge into their long term accommodation.  

The program interfaces with the NDIS as clients transition towards their goals and return to community. Clients are aged between 18-64 years, have access to onsite nursing, disability support workers and allied health professionals.  

We are recruiting this role on a permanent part time basis working 45hours  (3 days per week) per fortnight!

About you and what you will need: 

  • Be registered with the Australian Health Practitioner Regulation Agency (AHPRA)  

  • Have excellent interpersonal skills and the ability to communicate with people from diverse backgrounds and experiences 

  • Deliver a strong clinical assessment service 

  • Prescribe and maintain supportive equipment, technology, and seating 

  • Lead the services therapy assistants, providing support to deliver person-centred therapy programs  

  • Have a satisfactory National Police Clearance that is less than one year old 

  • Evidence of Flu vaccination  

  • Possess WA Drivers Licence  

  • First Aid certificate (less than 3 years old)
