This role involves overseeing the day-to-day coordination of a rehabilitation program tailored to the client’s with acquired brain injury, ensuring that it aligns with individual client outcomes. Key responsibilities include collaborating with the Oats St Program Manager and Roster Coordinator to develop rosters based on client needs and budget considerations. Additionally, the role coordinates client referrals, admissions, transfers, and discharges, while ensuring care and rehabilitation practices adhere to relevant codes of ethics, standards, and legislation.
Effective communication with clients, families, and colleagues is essential, with an emphasis on conflict resolution and problem-solving. The role also involves identifying areas for improvement, facilitating quality improvement activities, and acting as a change agent. Leadership duties include participating in staff recruitment, mentoring, supervising, and supporting team members, as well as assisting with budget preparation.
The position requires collaboration with the multidisciplinary team, internal departments, and external stakeholders to evaluate and improve the rehabilitation service model and to foster community-based client interactions. The coordinator also leads team efforts, facilitates communication, and promotes continuous improvement in the rehabilitation process.
