The Opportunity
We are seeking an experienced and passionate Rehabilitation Coordinator to lead the delivery and coordination of our rehabilitation program, supporting clients with acquired brain injury, neurological conditions, and complex medical needs.
This is an exciting opportunity for a clinician or rehabilitation professional who enjoys combining client-centred practice with leadership, service coordination, and team development.
This is a full-time, Monday to Friday position, working within our TRP (Transitional Rehabilitation program) based in Marangaroo.
Our TRP Program consists of 23 beds that provides continuous rehabilitation to clients aged 18-64 years referred from a metro hospital. The interdisciplinary team supports clients to work towards their goal and potentially to live in and integrate back to the community.
The service delivers an individualised, active support and person centred model through a multidisciplinary team, combining clinical support and rehabilitation programs while maximising independence and quality of life. It interfaces with the NDIS as clients work towards their goals and transition back into community living. The team includes nursing, disability support workers and allied health professionals.
Clients referred to this service a range of acquired disability including acquired brain injury, complex medical health needs and other co-morbities. This can include diagnosis of SCI, ABI, TBI, Stroke, Prolonged Disorders of Consciousness (PDOC), neurodegenerative conditions (eg. Huntington’s Disease, younger onset dementia) and other complex medical diagnosis.
